- Transcript of Record (TOR) valid for transfer (Original Copy)
- NSTP/CWTS 1/LTS Serial Number
- 2 pcs. PSA Birth Certificate (Xerox Copy)
- 2 pcs. PSA Marriage Certificate for married student (Xerox Copy)
- 3 pcs. 2x2 ID Picture with collar and white background
